How to create a form in Google Docs
Want to create a form in Google Docs and Spreadsheets? It’s easy. Check out the one I created in the tutorial below here. Create a new spreadsheet in Google Docs. Click the “Share” tab which is in the top right hand corner. Name and save the spreadsheet as “Form”.
A new page will load. Click “to fill out a formNEW” then “Start editing your form”.
A new page will open displaying the options that you need to complete in order to construct your form. In the screenshot below, I have named the form “Quick Survey”, filled in a short description for it, made the first question ask “What country do you live in?” and added some help text. The input style for this question is “text”. Save the question and then add another one.
In the next question, I want to find out the age of people submitting the form. The second question asks “How old are you?”. Instead of using a “text” input, I am using “multiple choice”. Select that accordingly and add the age range that takes your fancy. I added some help text too. Save. Add a new question.
In the final question I want to find out the gender of people. I want a “drop down” menu for people to choose from, hence I have selected “Choose from list”. Enter what you want people to select. It can be more than two options if required. See screenshot below. Once you have done that, click “Next, choose recipients”
Follow the instructions below and voila! There is your new form.
